Required application paperwork:

​1. Complete application.

2. Sign Record’s Release for THS to obtain the student’s records.
3. Take the diagnostic test. 
4. After these points have been completed, the acceptance decision will then
be made.
5. Copies of the child’s birth certificate and current immunization records
need turned into the office.
